一、依據「114年度教育部教學實踐研究區域基地計畫」辦理。

二、旨揭辦法係為協助教師撰寫教學實踐研究計畫，東區基地提供計畫撰寫諮詢服務，邀請諮詢顧問針對申請者研提計畫內容與方向給予建議。

三、申請期間：即日起至114年10月12日(日)止。

四、諮詢期間：自114年10月13日(一)至114年11月7日(五)止。
五、實施方式說明：
　(一)一對一線上諮詢，每次諮詢以1.5小時為限，諮詢費由本地核支。
　(二)諮詢服務由教學實踐研究計畫六校區域基地合作辦理，為減少Mentor負擔，並維持資源分配的公平性，每位Mentor的服務案件以3案為限，諮詢申請人以申請2次服務為限 (包含申請其他基地之諮詢服務) 。
　(三)全國大專校院符合申請教學實踐研究計畫資格之教師，優先錄取「東區跨校教師社群成員」及「東區基地聯盟學校教師」。
　(四)每位申請人申請諮詢服務至多2次，且申請人應回傳第1次諮詢紀錄回饋表後，始得申請第2次諮詢服務。
　(五)申請人須提出「115年度教育部教學實踐研究計畫」申請。

六、申請流程：
(一)請於114年10月12日(日)23:59前，寄送「申請表WORD與PDF檔(務必簽名)」至承辦人李小姐信箱mcli@niu.edu.tw。
(二)申請人提出申請後，東區基地將盡快通知媒合結果，申請表將提供諮詢 Mentor 參考，倘如已完成計畫書初稿亦請一併提供。
(三)申請人須於諮詢後一週內回傳「諮詢紀錄回饋表」

1. This application is conducted in accordance with the "2015 Ministry of Education Teaching Practice Research Regional Base Project."

2. This notice is intended to assist teachers in writing teaching practice research plans. The East District Base provides consulting services, inviting consultants to advise applicants on the content and direction of their plans.

3. Application Period: From now until October 12, 2015 (Sunday).

4. Consultation Period: From October 13, 2015 (Monday) to November 7, 2015 (Friday).

5. Implementation Methods:

(1) One-on-one online consultations, each limited to 1.5 hours. Consultation fees will be paid by local authorities.

(2) Consultation services will be provided collaboratively by the six regional bases of the Teaching Practice Research Project. To reduce the burden on mentors and maintain fair resource allocation, each mentor is limited to three cases, and applicants are limited to two applications (including those applying for consultation services at other bases).
(3) Faculty members of the East District Inter-school Teacher Community and teachers from East District Base Alliance schools who meet the eligibility requirements for the Teaching Practice Research Project will be given priority.

(4) Each applicant may apply for consultation services up to two times, and must submit a feedback form for the first consultation before applying for the second consultation.

(5) Applicants must submit an application for the "116 Ministry of Education Teaching Practice Research Project."

VI. Application Process:

(1) Please send the "Application Form (Word and PDF) (signed required)" to the organizer, Ms. Li, at mcli@niu.edu.tw, by 23:59, October 12, 2015.

(2) After submitting your application, the East District Base will notify you of the matching results as soon as possible. The application form will be provided to the consultant for reference. If you have completed a draft proposal, please also provide it.
(3) Applicants must return the "Consultation Record Feedback Form" within one week of the consultation.
