一、為協助教師撰寫教學實踐研究計畫，南區基地提供計畫撰寫諮詢服務，以線上一對一諮詢的方式針對計畫申請與撰寫內容給予建議。

二、實施方式：線上一對一諮詢或書面資料諮詢。

三、諮詢期間：114年11月17日（星期一）至114年11月30日（星期日）止。

四、申請資格：
(一)限全國大專校院符合申請教學實踐研究計畫資格之教師。
(二)申請人須提出115年度教育部教學實踐研究計畫申請。
(三)每位申請人諮詢服務至多以2次為原則，申請人應回傳第1次諮詢紀錄及回饋表後，始得申請第2次諮詢服務。
(四)優先錄取「南區跨校教師社群成員」、「南區區域大專院校教師（包含高雄、台南、屏東、嘉義、澎湖）」及「未曾獲核過教學實踐研究計畫之教師」。

五、申請流程：
(一)即日起至114年11月19日（星期三）23:59前，寄送申請表Word及Pdf檔(務必簽名)至承辦人吳小姐信箱：daiting2163@mail.nsysu.edu.tw (信件主旨：【教學實踐研究計畫南區區域基地】教學實踐研究計畫撰寫諮詢服務申請表_學校&申請人姓名)。
(二)申請經核准後，承辦人將寄發申請成功通知。如未收到來信，請主動聯繫承辦人確認郵件是否發送成功。申請成功後，南區基地將通知申請人並進行後續媒合安排。
(三)申請人須於諮詢後一週內回傳「教學實踐研究計畫撰寫諮詢紀錄及回饋表」。

1. To assist teachers in writing their teaching practice research proposals, the Southern District Base provides consultation services, offering advice on proposal application and writing content through online, one-on-one consultations.

2. Implementation Method: Online, one-on-one consultations or written consultations.

3. Consultation Period: November 17, 2025 (Monday) to November 30, 2025 (Sunday).

4. Application Eligibility:
(1) Limited to faculty members from universities and colleges nationwide who meet the requirements for applying for the Teaching Practice Research Project.
(2) Applicants must submit an application for the 2025 Ministry of Education Teaching Practice Research Project.
(3) Each applicant is limited to two consultations. Applicants must submit the first consultation record and feedback form before applying for the second consultation.
(4) Priority will be given to "Members of the Southern District Inter-University Faculty Community," "Teachers from universities and colleges in the Southern District (including Kaohsiung, Tainan, Pingtung, Chiayi, and Penghu)," and "Teachers who have not yet had a teaching practice research proposal approved."

V. Application Process:
(1) From now until 11:59 PM on November 19, 2025 (Wednesday), please send the application form (Word and PDF files, signed by the applicant) to the sponsor, Ms. Wu, at daiting2163@mail.nsysu.edu.tw (Subject: [Teaching Practice Research Project Southern Regional Base] Teaching Practice Research Project Writing Consultation Service Application Form_School & Applicant Name).
(2) Upon approval, the sponsor will send a notification of successful application. If you do not receive this email, please contact the sponsor to confirm the email was sent successfully. If your application is successful, the Southern Regional Base will notify the applicant and make subsequent matchmaking arrangements.
(3) Applicants must return the "Teaching Practice Research Project Writing Consultation Record and Feedback Form" within one week of the consultation.
